The Board asks that you take sponsorship seriously and asks that you fill out the below form to the best of your ability.

Either one or both sponsors should have visited the residence of the applicant.

It is strongly recommended that members not sponsor individuals whom they do not know personally, eg, persons known only through email, social media or written correspondence.
